SCOPE OF WORK: The COVID The Motor Vehicle Operator/Transporter & COVID Outreach Coordinator will provide transportation as scheduled to referred facilities per Indian Health Services.
Collaborate in coordination amongst Indian Health Services case management, CHR Program LPN and referred Hospital Case Managers to support and to ensure safest means of transportation for Standing Rock Sioux Tribe community members.
The Motor Vehicle/Transporter & Outreach Coordinator will enforce mitigation of COVID 19 procedures while transporting patients and provide education to mitigate COVID-19 Standing Rock Sioux Tribe communities at all CHR Program screening events.
DUTIES AND RESPONSIBILITIES:
Provide safe transportation to patients as scheduled.
Transport patient as referred to outside facilities and upon discharge per Case Management instruction and Tribal coordination plan.
Provide dialysis patient transportation while on isolation in accordance with Case Monitoring plan instructions.
Coordinate with Office Manager and LPN to provide rapid response as needed.
Assist in conducting rapid testing as scheduled for communities as needed.
Conduct delivery of supplies, PPE, and medications for patients isolated.
Coordinate directly with the CHR Program LPN, Indian Health Services Case Mangers to ensure patient transports are scheduled and completed.
Provide education to mitigate COVID-19 in Standing Rock Sioux Tribe communities at all CHR Program Health screening events.
Will also assist in all other duties as assigned.
QUALIFICATIONS:
High School Diploma/GED Certification with one (1) year of experience of providing safe transportation.
Valid driver’s license and meet insurability requirements.
Effectively communicate information both verbal and written; ability to understand and carry out both verbal and written instructions and request clarification when needed
Excellent organizational skills and strong attention to detail
Solid interpersonal skills and professionalism; maintain tact, courteous, composure, and flexibility in a stressful environment
Pass a back ground check
JOB REQUIREMENTS:
Ability to obtain certification to conduct COVID–19 Ag Card rapid antigen test.
Ability to obtain HIPPA certification and complete any training that is assigned by Tribal Public Health authority
Demonstrate effective knowledge on the COVID–19 Ag Card rapid antigen testing requirements.
Ability to operate CHR Program Vehicle in a safe and appropriate manner (pick-up/drop-off).
Ability to Sign and understand CHR program confidentiality statement with understanding of the penalties for any violations and adhere to HIPPA, Privacy Act and other medical records laws as they pertain to health information.
Ability to collaborate, network and establish contacts with area health service professionals, programs and community/local district officials.
Ability to multitask with desire to learn, take on incremental responsibility, and work in a team environment.
Ability to maintain cleanliness and disinfection of frequently touched surfaces and between each and at the end of each test.
Ability to lift 50lbs and be able to sit and stand for a long periods of time.
Ability to transport patients that are experiencing suicidal ideations due to COVID as a last resort to off reservation to out of state facilities to ensure wait time or loss of patient bed at outside facilities.
