SCOPE OF POSITION:
The Licensed Practical Nurse (LPN)/Community Health Worker (CHW) will be responsible to provide health care support and nursing assistance according to plan of care; conduct home visits (testing, nutrition education, recording vital signs, home therapy and basic short-term health care aide) for patients with coordination from discharge summary from a medical facility and/or Care Manager and provide support to clients in accordance with established policies and protocols of Standing Rock Sioux Tribe.
SUPERVISION: The Licensed Practical Nurse (LPN) will work under the direct supervision of the CHR Director /Program, who will provide guidance, direction, and conduct performance evaluations.
In addition will coordinate and collaborate closely with Indian Health Services Nursing and Public Health Nursing in assisting with administering tests, procedures and vaccinations.
DUTIES AND RESPONSIBILITIES: Develop and implement efficient and effective system to process patients through the program.
Conducts basic vital signs (weights, temperature, blood pressure, blood glucometer, oximeter) during home visits and chart properly on the PCC in order for the information to be transferred to the RPMS Responds to health concerns of the patient (new and on-going); establishes a line of communication between the provider and the patient and requests guidance from Clinic Supervisor as necessary.
Provide communication with patients regarding plan of care according to provider instructions, procedure preps and discharge instructions, reviews test results and medication refills within 24 business hours of contact.
Record and document all patient interactions in Electronic Health Record (EM\HR).
Accurately document medications, vital signs, pharmacy of choice, allergies, and directives into the patient’s EHR chart.
Maintain reporting requirements and documentation of CHW activity on the PCC forms, patient sign-in logs, and other required forms for administrative need and quality assurance purposes.
Preload patients’ charts in EHR after patients’ complete patient link online.
Conducts Navigation of Electronic Medical Record, Patient Portal, and email.
Provides patient roster documenting needs and submit to Director.
Sends and retrieves patient information as necessary Produces daily reports, sends letters to patients and faxes results/reports when necessary.
Conducts and assists with COVID-19 processes and procedures; testing, contact tracing, and vaccinations.
Assist with medication delivery to IHS Satellite (Wakpala, Rock Creek, Cannon Ball) Clinics.
Assist patients with any Health insurance or benefits issues or concerns.
Assists patients with completing medical history questionnaires.
Schedules and conducts home visits to members of the community within the service area in accordance with the policies and procedures of the CHR Program.
Coordinate with the medical clinic for any home visitation, follow up and requests guidance from Clinic Supervisor when necessary.
Conducts home visitation upon referral and/or ordered by the Clinic and/or requested by the patients.
Conduct patient and care giver education in regards to medications, treatments, procedures, preparations and issues of health maintenance under the direct supervision of the providers.
Provides necessary patient education in regard to procedure prep instruction; ensure patient has appropriate laboratory requisitions and prescriptions as ordered by provider and provides summary to patient.
Provide training and instruction on the proper use personal protective equipment (PPE).
Provide advocacy, education services and facilitate access to health services that may be available within the program.
Provides flyers and brochures regarding nutrition or any health related material to patients.
Facilitate and organize quarterly events and annual fair.
Maintain supplies and track inventory to ensure to supplies are ordered in a timely manner.
Performs all other tasks and duties within the scope of the CHR Program.
QUALIFICATIONS: Licensed Practical Nurse (LPN license) with two (2) years documented work experience in nursing field.
Strong computer skills with working knowledge of Microsoft Office Systems with emphasis in Word and Excel.
Effectively communicate information both verbal and written; ability to understand and carry out both verbal and written instructions and request clarification when needed.
Excellent organizational skills and strong attention to detail.
Solid interpersonal skills and professionalism; maintain tact, courtesy, composure and flexibility in a stressful environment.
Elevated level of ethical standards and maintain a high degree of confidentiality.
Pass a background check that includes a child abuse and neglect screening in accordance with applicable law and regulations.
Valid driver’s license and meet insurability requirements.
JOB REQUIREMENTS: Ability to obtain a Certificate of Eligibility to access computer systems and patient data and complete the Indian Health Services ITAC Form (Information Technology Access Control).
Demonstrate effective knowledge of Medical System, RPMS (Resource Patient Management System) or EHR (Electronic Health Record) Ability to obtain online CHW/CHR Certification, HIPAA (Health Insurance Portability and Accountability Act) and NARCAN Training within 90 days of hire.
Maintain CPR/First Aid Certification on an annual/yearly basis.
Ability to collaborate with clinic staff to ensure that optimal patient care and customer service is maintained.
Demonstrate knowledge effectively assist in the achievement of meeting our standards for excellence in patient care and customer service.
Sign confidentiality statement and HIPAA regulations with understanding of the penalties and violations; non-disclosure of private or protected health information unless required to do so in an official capacity and/or job function.
Ability to adhere to CHR Program Policies and Procedures.
Ability to actively participate in required meetings; communicates concerns to supervisor.
Demonstrate organizational and time management skills with ability to perform with minimum supervision; optimize clinic efficiencies.
Ability to collaborate with Data Entry staff to ensure records are imported into the EHR chart.
Ability to provide education to the patients when home visiting upon receipt of appropriate trainings by Health/Medical Consultant, Ability to assists in the training and orientation of new staff.
Demonstrate proficiency of data entry skills and Microsoft Office applications.
Ability to collaborate with other CNA’s and Community Health Workers.
Ability to serve as a liaison between the clinic and the patients.
Ability to multitask with continuous improvement mindset, desire to learn, take on incremental responsibility, and work in a team environment Ability to work under pressure while meeting deadlines Demonstrate cultural competency and sensitivity including cultural diversity Be able to lift a minimum of 25 pounds and be able to sit and stand for a long periods of time.
