The 1915i Care Coordinator is part of a multidisciplinary team who serves as the bridge across systems involving the educational system, behavioral health, criminal justice, housing, and human services for persons enrolled in ND Medicaid or Medicaid Expansion with income of 150% below poverty level, a qualified behavioral health diagnosis and has a qualifying WHODAS score.
The 1915i Care Coordinator primary role is to provide care coordination and supports to clients enrolled in the 1915i Program.
Coordinator ensures 1915i clients successfully achieve monthly outcomes.
The 1915i Care Coordinator will facilitate the service referral process which includes the direct internal and external referrals for housing, employment, legal, educational, peer and family support, and all other essential community-based resources and follow-through services.
The 1915i Care Coordinator will work directly with the Business and Finance staff to ensure proper Medicaid billing and the Director of Human Services for compliance with documentation, outcomes, and client assignments.
Position will be required to serve participants in 1915i Medicaid Programs and may include serving clients in the Community Connect program.
Required Qualifications:
Bachelor of Applied Science degree in criminal justice or human services related field.
Other types of bachelor’s degrees will be considered.
Must demonstrate previous competencies in care coordination and management practices.
Must have a valid driver license.
Must be able to pass a background check.
Preferred Qualifications
Completion of ND State required care coordination training.
Work experience in case management.
Prior program management experience.
Care Coordination Job Duties:
Provide care coordination and support services to the clients.
Meet with clients face to face once a month to develop a care plan.
Serve as oversight of the client’s success in meeting goals of the care plan for a multidisciplinary care team.
Enter care plan and case notes into Therap for each client every week.
Ensure proper billing codes are entered into Therap for accurate billing purposes.
Ensure clients engagement of supports take place at minimum of a weekly basis.
Work with the state program administrators to obtain approval for gap funding requests.
Track, report and submit monthly engagements and outcomes each month per guidelines.
Coordinate referrals for therapy or counseling, chemical health treatment, psychiatry, etc.
Examples of assistance to clients: make formal referrals to appropriate resources, schedule appointments to meet goals, attend appointments (as determined appropriate and only upon request made by client).
Coordinate transportation needs between peer support specialists and transportation drivers on behalf of clients.
Coordinate and assist clients with navigating essential community resources as needed.
Provide technical assistance for clients when necessary.
Maintain thorough communications with internal care coordinators and administrators, state program administrators and the systems of care: education system, human services, corrections, behavioral health (mental and chemical) providers and medical providers.
Respond to behavioral health crisis situations appropriately.
Attend industry-related conferences and trainings including community outreach.
Track client data and submit a weekly report to administration.
Participate in duties by Supervisor.
Administrative Care Coordination Job Duties:
Assist with the implementation of the 1915i (Medicaid) Program for organization.
Assist with the development and ongoing amendments of the program policy and procedures handbook.
Assign referrals to other Care Coordinators and Support staff.
Assist with the monitoring of clientele for the organization.
Ensure billing is coded correctly.
Conduct weekly team meetings with Care Coordinators.
Track additional data and report to administration on a weekly basis.
Participate in state meetings relating to 1915i programming.
