Clinical Nurse
PRIMARY OBJECTIVE: Responsible for working as a member of the patient's health care team in collaboration with the patient's primary care provider, pharmacist and pharmacy support staff.
To provide support with clinical visits by identifying patient's eligibility for care and execution of guided processes to complete the clinical visit.
Assess health care needs that require additional follow up by the primary provider and assists patients in scheduling appropriate follow up.
Performs all immunization duties and represents Thrifty White Pharmacy at immunization clinics, health fairs, and other events where immunizations are provided.
MAJOR RESPONSBILILTIES:
Serves as a patient advocate and assists in identification and improvement of service delivery
Works collaboratively with clinical teams to accomplish clinical services in an efficient manner for patient and team
Follows clinical protocols identified by the clinical team
Assists patients in connecting with community resources
Addresses open quality gaps with patients and work collaboratively with patients towards closure of those gaps
Appropriately documents within the electronic record to satisfy closed quality gaps
Consults with the clinical staff and ancillary department staff to eliminate barriers to the efficient delivery of care
Participates in regular team meetings, staff meetings, and quality improvement projects to improve patient care
Assists with other clinical duties such as prior authorizations, patient messages, benefits investigation, etc.
Maintain annual competencies for role.
MINIMUM QUALIFICATIONS:
LPN degree in Nursing
Current, valid state Licensed Practical Nurse license
Current CPR certification
Two years of nursing experience in primary care, community nursing, critical care, or home health is preferred.
Knowledge, Skills, and Abilities
Ability to work independently and exercise clinical judgement in interactions with physicians, payers, and patients and their families
Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components
Ability to communicate effectively verbally and in writing
Ability to maintain patient, team member, and employer confidentiality.
Compliance with all HIPAA regulations
Ability to use a computer and electronic medical record
Ability to work as a team
