Duties: Duties are determined by the facility and may include, but are not limited to the following: 1.
Facilitates admissions/discharges of residents; 2.
Maintains contact with family/resident to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning; 3.
Develops and maintains social care plan/psychosocial assessment; 4.
Arranges supportive services if needed; 5.
Educates staff regarding building relationships with residents in order to better serve the resident’s individual needs; 6.
Develops and maintains policies for resident conduct and behavior; 7.
Public relations responsibilities, including public education; 8.
Provides assistance to residents for maintenance of each resident’s ability to control every day physical needs; and mental and psychosocial needs; 9.
Consults with allied professional health personnel regarding provisions for the social and emotional needs of the resident and family.
10.
Develop, maintain, and review the department’s policies, procedures, job descriptions, etc.
at least annually making revisions as necessary or recommendations for change to Administrator.
11.
Assure prompt resolution of complaints and grievances from staff, residents and visitors and make oral or written reports to the Administrator and other agencies as required.
12.
Assist with driving residents to appointments or activities when necessary.
13.
Assist with resident activities as needed and necessary.
14.
Active participation in QAPI.
Requirements: - A qualified social worker in the state of North Dakota or a Social Services Designee with a high school diploma or GED receiving quarterly on site consultations with a qualified social worker.
- Minimum of 5 years of experience in social services, with at least 2 years in a supervisory role - Must have the desire to work with convalescent, geriatric or light psychiatric residents.
Will assure that the resident's rights to fair and equitable treatment, self determination, individuality, privacy, property, and civil rights, including the right to wage complaints, are well established and maintained at all times.
Must maintain confidentiality of resident and facility information.
The employee shall use only the minimum necessary information to perform his or her responsibilities.
