Hospice and Visiting Nurse Services Coordinator
Full-time (36 hours/week).
Weekday, day shifts.
Department call rotation for evenings, holidays, and weekends.
The Hospice and Visiting Nurse Services Coordinator is a supervisory position with the skills and knowledge of a registered nurse.
This role is responsible for planning, coordinating, and implementing the care of Hospice and Visiting Nurse Service patients.
Promotes the growth and development of the Hospice/ Visiting Nurse departments, ensuring compliance with Federal and State regulations.
Must be able to work with terminally ill patients and their families to help them effectively cope with dying.
Experience in Hospice is preferred.
Prior experience in supervisory capacity preferred.
Demonstrates knowledge in caring for patients and families in death and dying situations.
Understands and possesses knowledge of chronic pain management.
Possesses an understanding of Hospice concepts and supports the value of palliative care.
Able to work with an interdisciplinary team to provide bereavement services.
Responds effectively in emergency and stressful situations.
Communicates frequently with coworkers, physicians, patients, and families.
Ability to arrange and rearrange daily schedule, taking into consideration the duration and location of scheduled visits, meetings, unexpected situations, etc.
Reviews and completes chart audits.
Is comfortable with independent decision-making and discussing death and dying issues.
Keeps current in advances in Hospice and Visiting Nurse Services by attending continuing education programs and in-service as allowed by hospital finances.
ESSENTIAL FUNCTIONS, ROLES, AND RESPONSIBILITIES Responsible for the coordination of daily operations of the Hospice and Visiting Nurse departments.
Responsible for assisting with the hiring and orientation of new employees.
Accomplishes staff results by communicating job expectations, planning, monitoring, and appraising job results.
Provides outreach, community education, social support, and advocacy to the patient/family as a component of team-based care.
Serves as a resource for patient/family to facilitate access to services and improve the quality and cultural competence of service delivery.
Collaborates with providers, nurses, therapists, and ancillary services to provide or assign duties for in-home care and services for patients with identified medical or psychological needs.
Ensures adherence to quality standards and customer service.
Ensures Hospice and Visiting Nurse Services departments comply with Federal and State regulations, maintaining licensure, and certification.
Prepares for and assists with surveys by regulatory bodies.
Submits monthly reports to Administration showing the number of patients served, admitted, discharged, etc.
Prepares special reports as needed.
Coordinates and supervises Hospice/ Visiting Nurse billing.
Supervises volunteer coordinator and volunteer involvement in the Hospice program.
Coordinates and supervises the Hospice fundraising program.
OTHER DUTIES Ensures the orientation process is completed within three months of hiring.
Develops, maintains/implements policies and procedures, and establishes departmental standards.
Provides consultation and guidance to personnel while promoting good employee relations.
Assures efficient, cost-effective use of supplies and equipment.
Represents the Hospice and Visiting Nurse departments with internal/external meetings.
Represents the Hospice department in organizational and community groups, when designated, to promote services offered and to educate the public on the Hospice concept and philosophy.
Facilitates Advisory Board meetings.
Conducts Hospice & Visiting Nurse staff meetings.
Updates contracts with contracted services as needed.
Responsible for assuring required reports are completed within designated time frames.
Submits monthly census/statistics reports to Administration.
Prepares special reports as needed.
Assists with all month-end reports and charge slips to ensure that all month-end paperwork is turned in to the Business Office by afternoon of the fifth working day of the following month.
Responsible for the establishment of quality assurance studies with appropriate follow up.
Develops indicators for assessing quality patient care and utilization review.
Upholds the mission and philosophy of Sakakawea Medical Center.
