00 Department Details The Care Management Team is looking for an RN to join their team in a Clinical Strategist role! Opportunity for growth and to impact change within the health system.

Job Summary Assists with the coordination of patient care across the continuum by collaborating with physicians, advanced practice providers, staff and other professionals.
Utilizes a population health perspective to guide care across the continuum and support patient care transitions.
Serves as a leader by demonstrating independence, assertiveness, critical thinking, problem solving skills, and expertise in ambulatory quality measures.
Is a liaison with the healthcare team, and serves as a link to community resources.
Accountable to oversee assigned entity compliance related to specific certifications.
Will also be accountable to Department of Health (DOH) statutory requirements, Center for Medicare and Medicaid (CMS) Conditions of Participation and standards of The Joint Commission (TJC) as applicable.
In addition, serve in a facilitation role to support and coordinate with those within the organization/entity who have primary responsibility for additional services.
Leads committees and task forces formed for the purpose of initial survey compliance and ongoing survey readiness as well as focus on the ongoing needs of the Medical Home program, standardization of staffing, roles, creating standard work for assigned roles, identifying/implementing standard interventions, training, and quality impact.
Coordinates efforts of additional clinical and support roles to include health and safety, performance improvement, quality, infection prevention and control, compliance and entity clinical services roles to meet DOH, CMS and TJC compliance needs.
Works closely with entity leaders to ensure operations are designed and implemented to support accreditation and certification compliance standards.
Manages the accrediting program, medical home program, team care, care management, and direct the care management process across the enterprise.
Must be self-motivated, self-directed, and autonomous; willing to teach the healthcare team; and seeks professional development in area of expertise.
Able to facilitate patient advocacy groups.
Knowledgeable about reimbursement for services provided.
Provides specialized, multidisciplinary patient and family centered education and coordinates off-site education opportunities as needed.
Supports quality improvement and transformation of the health care delivery system.
Understands population health.
Demonstrates skill in accessing, preparing and using information as relevant to position.
Verbalizes and demonstrates knowledge of procedures for maintaining security, confidentiality, and integrity of patient, family and other medical information.
Demonstrates ability to collect, analyze and present data as appropriate to position.
Qualifications Bachelor's degree in nursing is required.
Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Master's degree in nursing or a health related field preferred.
Three years experience in regulatory compliance, quality management, and performance improvement or in a management or leadership position in healthcare preferred.
Experience in ambulatory care, care coordination or case management desired.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
Obtains and subsequently maintains required department specific competencies and certifications.
Requires current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position.
Specialty certification and knowledge in an appropriate field is required, for example, but not limited : Certified Case Manager (CCM), Healthcare Quality, Healthcare Accreditation Certification, Joint Commission, or within ambulatory care.
