*MAJOR RESPONSIBILITY: *Under the supervision of the Health Maintenance Manager, this position will provide Health Maintenance services to senior citizens in Burleigh County.
Services are provided at the Burleigh County Senior Center, Monday – Friday and at satellite clinics (Wing/Sterling) as determined by the Health Maintenance Manager.
This position can be part time or full time, depending on the needs of the successful applicant.
*RELATED DUTIES:* 1.
Maintain a system of cooperation and referral between the Health Maintenance program and the Outreach Coordinator, the Home Delivered Meals Coordinator, community medical facilities, and community health, education and welfare agencies.
2.
Maintain records as required.
3.
Follow all guidelines as established in the “State and Community Programs Funded under the Older American Act Policies and Procedures, Service Chapter 650-25.
” 4.
Set up regularly scheduled health maintenance programs to provide: a.
Blood pressure screenings b.
Blood sugar screenings c.
Foot care d.
Respond to client questions regarding health and make appropriate referrals to physician and/or agency 5.
Provide home visits in Bismarck area to home bound senior citizens in need of services described in item 4.
6.
Conduct bi-monthly health maintenance clinics and home visits at the rural sites of Wing and Sterling in Burleigh County as assigned.
7.
Receive referrals from other personnel within the program and other agencies, both public and private.
8.
Follow cleaning/sanitation procedures for clippers, curettes, and other tools used for health maintenance services.
9.
Maintain a clean and professional office.
10.
Participate in training opportunities as assigned.
11.
Perform all other duties related to health as assigned by the Health Maintenance Manager or Executive Director.
*MINIMUM REQUIREMENTS:* A degree in Nursing and a North Dakota RN licensure, with training and interest in gerontology or public health nursing.
Must be empathetic toward the elderly and maintain the highest level of professional standards and ethics.
Must have valid driver’s license.
Pay depends on experience and level of degree.
Job Types: Full-time, Part-time Pay: $28.
